Please note: To assure that you will get your desired scheduled time, all 9am appointments must be scheduled by 5pm the previous day.

Appointment Preparation
If this is your first visit to our massage therapy school, please arrive at least 15 minutes early to fill out the required paperwork. Or, you can complete the necessary paperwork prior to your appointment by printing and filling out the following forms: Health History Form and HIPAA Privacy Statement. Just don't forget to bring them with you!
